Our Donors

Hearts & Minds Patient Advocates is a 501(c)3 not-for-profit company.  We need donors like you to help us to support the hundred of patients and their families that need support during one of the most stressful times of their lives.  A healthcare crisis can happen to anyone at any time.

Donors to Hearts & Minds Patient Advocates include a variety of sources: 

  • Individuals: This includes general members of the community, but also a significant portion comes from "grateful patients" and their families who have personally experienced transformative care and wish to express their gratitude.

  • Hospital Employees and Physicians: Staff members may also contribute to the betterment of their workplace and patient programs through donations or payroll deductions. 

  • Corporations: Businesses may provide charitable contributions, sponsor events, or partner with a with us to bring more awareness to the need for Patient Advocates.

  • Foundations and Grant Organizations: We align with all public and private Foundations and Granting Organizations that have a mission for supporting the patient care and advocacy.

  • Communities: Faith-based and Community organization can make contributions from proceeds from fundraisers or donations in memory of a patient to support our patient advocacy.

The Listener
 

Up to $49

Funds essential daily operation costs like communication tools (phone/video conferencing) and travel stipends, ensuring an advocate can make first contact and listen to a client's story.

The Guide
 

$50 - $249

Covers the cost of one-on-one consultation time (approx. 2–5 hours) to help a low-income family navigate complex medical bills, paperwork, and resource applications.

The Care Coordinator
 

$250 - $999

Sponsors one full client case initiation, including the initial assessment, drafting of action plans, and coordination with a client's key providers (doctors, hospitals, social workers).

The Patient Champion
 

$1,000 - $4,999

Provides funding for the specialized training of a new volunteer advocate in complex areas like financial management and will planning for the critically ill, as mentioned in your scope.

The Hearts & Minds Founder

$5.000+

Supports the development of a new program area, such as expanding advocacy services to a new underserved demographic (e.g., dedicated post-partum resources for expecting mothers) or securing vital technological infrastructure.

Administrative & Technology

Office Supplies: Paper, printer ink, file folders for client documentation. Software/Subscriptions: Cloud storage, communication platform licenses, or specialized medical records management software.

Support Group

Advocate Support

Transportation Vouchers: Gas cards or ride-share credits to help volunteer advocates travel to client homes or appointments. Professional Development: Complimentary slots for staff/volunteers at training seminars related to HIPAA, geriatric care, or motivational interviewing.

Client Support

Translation Services: Pro-bono translation services for documents or meetings with non-English-speaking clients. Wellness & Respite: Gift certificates for healthy meals, transportation, or short-term relief services for clients or their primary family caregivers.

Pro-Bono Services: Legal consultation (e.g., reviewing consent forms), accounting/bookkeeping services, or marketing/website design. Meeting Space: Access to conference rooms or quiet office spaces for client intake and confidential meetings.

In-Kind donations are non-cash contributions of goods or services that directly support the mission of providing advocacy and coordination. These are especially valuable for a nonprofit focused on logistical support and communication.
